Membership services are not submitted to insurance.
Patients receiving services through Reach+ should not alternate the same membership-covered service between self-pay membership billing and insurance billing.
Patients preferring insurance-based care may instead schedule through Outreach Health when medically appropriate.
Cancellation
- • No new credits accrue after cancellation.
- • Previously earned credits remain available until their original expiration date.
- • Credits remain subject to the six-month expiration policy.
- • Membership fees are generally non-refundable once charged.
- • Credits cannot be exchanged for cash.
- • Membership benefits may change with advance notice.
